Insurance coverage and in-network status can vary by plan and provider. We recommend calling us before your appointment so we can help you understand your options.

Self-pay pricing

For patients without insurance, or for services not covered by insurance:

First visit

Follow-up visit

$80

  • No membership fee
  • No recurring monitoring or membership charges

Point-of-care laboratory testing

Point-of-care testing is available for an additional $60, including:

  • Hemoglobin (Hb)
  • HbA1c
  • Random glucose
  • Lipid panel

Additional laboratory tests or services, when medically necessary, may have separate charges.
